Request for Quotation for "Provision of Medical supplies for intensive care for Covid-19 severely affected patients in Nuwakot"

Shanti Volunteer Association (SVA) Nepal is a Japanese NGO partnership with Community And Rural Development Society Nepal (CARDSN), a Nuwakot based NGO implementing a emergency project "Provision of Medical supplies for intensive care for Covid-19 severely affected patients in Nuwakot” based on agreement with SWC. SVA Nepal/CARDSN seeks to invite interested and qualified companies with proven capacity for the supply of Medical equipments in package.

Documents to be submitted (Please submit the latest documents wherever applicable):

  1. Organizational Profile with experience
  2. Cover letter mentioning the bank details for the payment of purchase with Cost and quantity of each medical equipment as per technical specification and warrant condition of each items, Delivery timeline and delivery cost to 2 hospitals in Nuwakot District using Format A signed and stamped in PDF Version .
  3. Copies of updated Legal documents (PAN/VAT certificate, Tax clearance certificate, Organization Registration)

Submit the Quotation via email with the subject line mentioning "Quotation for the supply of Medical equipments" to the following email covidprocurement2021@gmail.com The deadline for submission of Quotation is 27th June 2021 no later than 5:00 pm.

Please Note:

  1. This Request is for package quotation meaning we accept the company who can supply all the mentioned medical equipment.
  2. Only short-listed companies will be notified for the further process. Number of equipment to be procured is subject to change.
  3. SVA Nepal/CARDSN reserves the right to accept or reject any or all application without giving any reasons whatsoever. Attempt for influence by any means during the selection process will lead for disqualification.
